Notice: AS THE STATE OF GEORGIA HAS NOT ADOPTED EDUCATIONAL AND TRAINING STANDARDS FOR THE PRACTICE OF HYPNOTISM, THIS STATEMENT OF CREDENTIALS IS FOR INFORMATIONAL PURPOSES ONLY. Hypnotism is a self-regulating profession and its practitioners are not licensed by state governments. I am not a physician nor a licensed health care provider and may not provide a medical diagnosis nor recommend discontinuance of medically prescribed treatments. If a client desires a diagnosis or any other type of treatment from a different practitioner, the client may seek such services at any time. In the event services are terminated by a client, the client has a right to coordinate transfer of services to another practitioner. A client has a right to refuse hypnotism services at any time. A client has a right to be free of physical, verbal or sexual abuse. A client has a right to know the expected duration of sessions, and may assert any right without retaliation.
Redress: We are certified members of the National Guild of Hypnotists, and practice in accordance with its Code of Ethics and Standards. If you have a complaint about my services or behavior that I cannot resolve for you personally, you may contact the National Guild of Hypnotists at P.O. Box 308, Merrimack, NH 03054-0308, (603) 429-9438, to seek redress. Other services than my own may be available to you in the community. You may locate such providers on the internet.
Fees: The charge for services vary and are subject to change at the free will of Fleurish Holistic Wellness, LLC. (FHW).

Late Arrivals: If you anticipate being late for your appointment, please notify us as soon as possible. While we will make every effort to accommodate late arrivals, please note that the session may need to be shortened to avoid impacting subsequent appointments.
Confidentiality: FHW will not release any information to anyone without a written authorization from you, except as provided for by law. You have a right to be allowed access to our written record about you.
Insurance: We suggest you think of our services as something that you will pay for personally. That will both protect your privacy and help you value the work you are doing more. In general, insurance companies do not like to cover hypnotic services, and we caution you not to expect them to do so.
1. Rescheduling:
Clients may reschedule their appointments online through our website www.fleurishholisticwellness.com. We understand that life can be unpredictable, and we strive to accommodate your needs as best as possible. However, we kindly request that you provide at least 24 hours' notice for any rescheduling requests.
2. Cancellation:
Appointments canceled with less than 24 hours' notice may be subject to a cancellation fee equivalent to one third (33.333%) of the session fee. We understand emergencies and unforeseen circumstances may arise, but late cancellations can disrupt our schedule and affect other clients. Therefore, we encourage you to notify us as soon as possible if you need to cancel your appointment.
3. Refunds:
Refunds will be provided for canceled appointments if the cancellation is made within the specified timeframe (at least 24 hours' notice). Refunds will be processed using the original method of payment. Please allow up to 14 business days for the refund to reflect in your account.
4. No-Show Policy:
Clients who fail to show up for their scheduled appointments without prior notice will be considered a "no-show." In the event of a no-show, the full session fee will be charged, and future appointments may require prepayment.
5. Communication:
Effective communication is essential to maintaining a smooth appointment scheduling process. If you need to reschedule or cancel your appointment, please do so promptly to allow us to adjust our schedule accordingly and accommodate other clients.
